Press secretaries are senior advisors (usually to politicians) who provide advice on how to deal with the media, and help them to maintain a positive public image and avoid negative media coverage. They often, but not always, act as the organisation's senior spokesperson. Many governments have press secretaries, although by far the most widely known is the White House Press Secretary.
